Request for Financial Assistance from CSVA - Individual Form

Senior volleyball players residing in Colorado may request financial assistance from CSVA prior to attending age 50 and over senior divisions of CSVA-approved national or international tournaments held in the United States.

You MUST submit this form BEFORE attending a national tournament for which you are requesting assistance. If you plan to participate in more than one tournament, you should use a separate form for each event. Support is available only for senior (50+) Colorado residents who actually participate in the event.

Assistance is limited to one team per division per person, up to two divisions per event, provided that the playing dates are different for each division.
